Upload your form and documents to Ahpra

We have an online upload service to receive applications, forms and supporting documents, so you don’t have to send these to us by hardcopy mail. The online upload makes it quicker and easier for you to send us the documents we need to assess your application.

Where indicated on your form, you must continue to supply a certified copy of your document (certified as true copy of the original document). For more information, see certifying documents

Scan your certified documents and save them as a file ready for upload. Your documents must be: 

  • clear and legible
  • less than 10MB, and
  • in one of the following formats: .JPG, .JPEG, .GIF, .PNG, .PDF, .TIF, .TIFF.

You must ensure you have all your files ready before you begin as you cannot save and resume your upload. Before you start uploading, make sure any forms or applications are complete and that you have all your supporting documents saved and ready to upload together. You can upload multiple files in one go.

Any applicants or registrants who wish to submit a form to Ahpra relevant to a profession.

Please do not post your form or certified documents. This will delay your application.

We have an online upload service, but some old forms may still be in circulation. So, even though your form says scanned copies will not be accepted, you need to submit your form and any relevant certified copies of your documents using the online upload.

We will contact you if there are any issues with your uploaded documents.

We do not accept cheques, money orders or bank drafts. You can pay securely by credit or debit card. We accept Visa or Mastercard.

Your documents must be in one of the following formats: .JPG, .JPEG, .GIF, .PNG, .PDF, .TIF, .TIFF.

Each file must be less than 10MB. You can upload more than one file per upload.

Submitting your application via mail will cause a delay in us receiving and assessing your application.

We will receive your application quicker if you use the Online upload service. However, if you have a complete application and need to submit it via mail, please ensure you do not submit any original documents as we will not be able to return these to you. Instead, original certified copies of documents must be attached to your application.
